SCRA INCIDENT REPORT FORM
Clubs should use this form to record any incidents, accidents and ‘near misses’.
Incidents should be recorded as soon as practicable after the event has occurred.
Clubs should retain records of such events and advise SCRA to ensure that lessons are learned and communicated throughout the Association.
